I never dig out old grass , just poison mow two weeks later to the lowest possible setting , throw a dressing of 25mil over the top and lay new turf , usually double the material costs for the labour portion. Eg 100m2 turf around $1000 for sir Walter and $200 for soil base & $1200 labour . $2400

I like the idea of no excavation and waste removal, but before laying the new topsoil, is it not standard to lay down slow realease fertiliser and preemergant to deal with dormant seeds so the new turf isn’t inundated with old lawn and weeds?

Depends on how fussy you want to be , I’ve done heaps and never had any complaints about old grass or weeds coming back through.
I usually fertilise on top with a no.20 or pasture booster , and keep the mowing height up high .
